Juniper is a statewide network of organizations delivering evidence-based programs to help adults live well, get fit and prevent falls.  Since 2018, 130+ organizations have joined this growing network to achieve better health in communities across Minnesota.  A core theme for Juniper participants is that you can take charge of your health and live well, even with challenges.  Through a networked approach, Juniper fosters collaboration among health plans, healthcare providers, housing providers, and community-based organizations that are dedicated to improving the health of the communities we serve — together.

Objectives:

  • Understand the impact and prevalence of chronic health conditions and falls in older adults, and how the Juniper Network is meeting the needs of the growing aging demographic.
  • Understand the variety of evidence-based programs offered through Juniper, and how the research supports achieving better care, better quality, lower costs (Triple Aim).
  • Understand the structure of the Juniper Network and identify how you can be involved as a referral source, leader, and/or sponsoring organization.

Sarah Shepherd, LSW, MPH, and Georgia Lane, MA

Georgia Lane is a Senior Program Developer with the Juniper network in Duluth, MN, working on building an integrated delivery network of health promotion programs. Her work focuses on the expansion of evidence-based health promotion programs and fostering collaboration among health systems and community-based organizations.  Prior to her current position she worked with Presbyterian Homes and Services in the Twin Cities to develop mobile primary care and care coordination services.  She also led the St. Paul ACT on Alzheimer’s coalition. Georgia has a MA in Social Work from the University of Minnesota.

Sarah Shepherd, LSW, MPH is a Program Developer with the Juniper network in the Twin Cities and Southeastern Minnesota since October 2018. As a Licensed Social Worker, she supported older adults and family caregivers for 13 years. in the skilled nursing facility, hospital, and community settings. Sarah graduated from the University of Minnesota’s Public Health Administration and Policy program in Spring 2018. Her professional goals are to improve collaboration between the healthcare system and community organizations to address social issues and health behaviors to improve the health of older adults, family caregivers, and their communities.
